Guardianship Forms can be accessed on this site.
Periodic reports are required every 12, 24 or 36 months (see order appointing guardian to determine reporting period) from your appointment date as guardian. Listed below are the titles of the typical reports that are needed.
File original documents with the Clerk.
File the following original documents/reports with the Clerk's office (the Clerk can file-stamp a copy for your records if you provide one at the time of filing):
*DO NOT file supporting financial documents (e.g. bank statements) with the Clerk. They must be submitted along with the Orders to the GMP. Guardians are required to retain copies of the supporting documents for four years in case the GMP needs the documents re-submitted for subsequent reviews.

Submit a bench copy of your periodic report along with originals of proposed orders and supporting financial documents to GMP.
Original proposed Orders must be submitted to the GMP (if you want a copy of the Orders after they have been signed, please provide a copy of the Orders along with a self-addressed stamped envelope):
