Submission of Periodic Reports

Benton and Franklin Counties require the use of our local court guardianship forms.


All Guardianship Forms can be accessed on this site.


Periodic reports are required every 12, 24 or 36 months (see order appointing guardian to determine reporting period) from your appointment date as guardian.  Listed below are the titles of the typical reports that are needed.


File original documents with the Clerk.


File the following original documents/reports with the Clerk's office (the Clerk can file-stamp a copy for your records if you provide one at the time of filing):

  1. Periodic Personal Care Plan (Form #22A)
  2. Guardian's Report, Accounting, and Proposed Budget (Form #29A) *


*DO NOT file supporting financial documents (e.g. bank statements) with the Clerk.  They must be submitted along with the Orders to the GMP.  Guardians are required to retain copies of the supporting documents for four years in case the GMP needs the documents re-submitted for subsequent reviews.

Submit originals of proposed orders and supporting financial documents to GMP.


Original proposed Orders must be submitted to the GMP (if you want a copy of the Orders after they have been signed, please provide a copy of the Orders along with a self-addressed stamped envelope):

  1. Order Approving Guardian Report, Accounting and Budget (Form #29B)
  2. Order Approving Personal Care Plan (Form #33C)
  3. **Also provide supporting financial documents (e.g. bank statements) to the GMP.

Ex Parte

Important Notice

During the month of December, the Court will conduct ex parte in courtrooms.  Click HERE for a description of the process.

Importance Notice

Trials will not be held during the  week of April 10, 2018 due to Judicial Spring Conference.
